Forms to View or Discuss a Student Conduct File or Case
- Copy of File Request Form
- This form is used to request a copy of a student's or student organization's conduct file. A student's or student organization's conduct file can only be released to someone other than the student or president of the student organization if that student or the student organization has a Waiver of Confidentiality on file.

- Waiver of Confidentiality
- This form is used by a student or student organization to waive his/her/its right of confidentiality and privacy to his/her/its conduct file and grant permission to a specific individual to discuss the student's or student organization's conduct case or conduct file.
Academic misconduct forms for Faculty
- Discipline Referral
- This form is used by faculty to request Student Conduct to examine and possibly resolve an allegation of academic misconduct (cheating and/or plagiarism) by a student or students.

- Faculty Disposition
- This form is used by faculty in academic misconduct situations (cheating and/or plagiarism) when the student does not dispute the academic misconduct allegation and the student accepts the academic sanction proposed by the instructor.
