Undergraduate Student Financial Aid Forms

Below is a list of commonly used forms and a description of their purpose. Read the instructions on each form carefully and submit it with the required documentation.

Name of Form	Purpose of Form
Budget Appeal 2013-14	Please note: The 2013-14 Budget Appeal Form will be available in August 2013. This form is used to claim allowable expenses that you are incurring that are greater than the standard budget. Note: The processing time for budget appeals is 2 to 4 weeks.
California Dream Act Application 2013-14	This form is used to apply for Berkeley Undergraduate Dream Act Aid for the 2013-14 academic year.
Eligible Non-Citizen Statement 2013-14	Only use this form if you have been directed to do so by a UC Berkeley Financial Aid and Scholarships Office representative. If you are required to fill out this form, you need to submit it in person to our office.
Free Application for Federal Student Aid (FAFSA) 2013-14	This form is used to apply for financial aid for the 2013-14 academic year. The last day to file a FAFSA for 2013-14 is March 15, 2014.
Ineligible PLUS Borrower Form 2013-14	Submit this form only if you were unable to complete the online PLUS application because of your non-citizen status, a federal loan default, or a grant overpayment. Do not use this form if your credit was denied; see Options If Credit Is Denied.
Parent Contribution Appeal Form 2013-14	This form is used to appeal the amount of the expected parent contribution. Read the form carefully before submitting.
Request for Confidential Information 2013-14	This form is used to request copies of documents that contain confidential information.
Student Contribution Appeal Form  2013-14	This form is used to appeal the amount of the expected student contribution. Read the form carefully before submitting.
Supplemental Foreign Income Information 2013-14	Only use this form if you have been directed to do so by a UC Berkeley Financial Aid and Scholarships Office representative. It is used to report foreign income.
U.S. Citizenship Confirmation 2013-14	This form is used when proof of U.S. citizenship is required. Only use this form if you have been directed to do so by a UC Berkeley Financial Aid and Scholarships Office representative. If you are required to fill out this form, you need to submit it in person to our office.
