2009-2010 UNDERGRADUATE FINANCIAL AID FORMS
Below is a list of commonly used forms that you can download, print, and fill out. Each one includes a description of its purpose. Read the instructions on the form carefully and submit wixth required documentation (when appropriate) to the Financial Aid Office.

Name of Form	Explanations
Budget Appeal	Download this form to appeal for an adjustment to the standard cost of attendance. In most cases, this will result in a larger loan offered to you.
Certification of Independence	This form is only used when presenting proof of independence through marriage, age, veteran status, having a legal dependent, and orphan/ward of the court status.
College Enrollment Verification	Only use this form if you have been directed to do so by the UC Berkeley Financial Aid Office. This form is used to help you document family members in college.
Denied Plus Form	Only use this form if you have been directed to do so by a UC Berkeley Financial Aid Office representative.
Eligible Non-Citizen Statement	Only use this form if you have been directed to do so by a UC Berkeley Financial Aid Office representative. If you are required to fill out this form you must bring it in person to our office.
Federal Loan Discharged Due to Disability	Download this form only if it appears as a "missing document" on your on-line offer letter.
Federal Student Loan Default Clearance Form	Download this form only if it appears as a "missing document" on your on-line offer letter.
FERPA Release Form	This form authorizes someone other than yourself to have access to your financial aid information.
Financial Aid Change Form	You can use this form to report various changes in your enrollment, residency, co-op program participation, or veteran's benefits. This form is also used for name changes.
NSLDS Record Not Matched to Name or Birthdate	Download this form only if the words on the left appear as a "missing document" on your on-line offer letter. If your missing document is "NSLDS Review," you don't need to take any action. The Financial Aid Office will clear this for you.
Off-Campus Housing Verification Form	Download this form only if you have been instructed to do so by a Financial Aid Office representative.
On-Time Appeal Form	Use this form only if you file a FAFSA after July 15, 2009.
Parent Contribution Appeal Form	This form is used to appeal the amount of the expected Parent Contribution. Read the form carefully before submitting.
Repayment Owed on Federal Funds Clearance	Download this form only if it appears as a "missing document" on your on-line offer letter.
Robert C. Byrd Scholarship Eligibility Form	Only use this form if you have been directed to do so by the UC Berkeley Financial Aid Office. This form is used to help verify eligibility for non-FAFSA filing Robert C. Byrd award recipients.
Satisfactory Academic Progress Appeal	You only use this form if the Financial Aid Office has notified you that you are out of compliance with the Satisfactory Academic Progress guidelines. If you aren't sure if this applies to you, contact the Financial Aid Office.
Selective Service Compliance Form	Only use this form if you have been directed to do so by the UC Berkeley Financial Aid Office. It is used to confirm your registration with Selective Service.
Social Security Number Certification	Only use this form if you have been directed to do so by the UC Berkeley Financial Aid Office. It is used to confirm that the name you are using for financial aid purposes matches the name reported to the Social Security Administration.
Student Contribution Appeal Form	This form is used to appeal the amount of the expected Student Contribution. Read the form carefully before submitting.
Supplemental Foreign Income Information	Only use this form if you have been directed to do so by the UC Berkeley Financial Aid Office. It is used to report foreign income.
U.S. Citizenship Confirmation Form	This form is used when proof of U.S. citizenship is being required. Only use this form if you have been directed to do so by a UC Berkeley Financial Aid Office representative.
Veterans Educational Benefits Reporting Form	Complete this form to report all Veterans Educational Benefits that you will be receiving in the 2009-10 academic year.
