Budget Appeal

This form is used to claim allowable expenses that you are incurring that are greater than the standard budget.
Eligible Non-Citizen Statement
Only use this form if you have been directed to do so by a UC Berkeley Financial Aid Office representative. If you are required to fill out this form you must bring it in person to our office.
Federal Student Loan Default Clearance Form Download this form only if it appears as a "missing document" on your on-line offer letter.
Selective Service Registration Verification Form Only use this form if you have been directed to do so by the UC Berkeley Financial Aid Office. It is used to confirm your registration with Selective Service.
Social Security Number Certification Only use this form if you have been directed to do so by the UC Berkeley Financial Aid Office. It is used to confirm that the name you are using for financial aid purposes matches the name reported to the Social Security Administration.
U.S. Citizenship Confirmation This form is used when proof of U.S. citizenship is being required. Only use this form if you have been directed to do so by a UC Berkeley Financial Aid Office representative.
Verification of Family Members in College
Only use this form if you have been directed to do so by the UC Berkeley Financial Aid Office. This form is used to help you document family members in college.
