Graduate Student Financial Aid Forms

Below is a list of commonly used forms that you can download, print, and fill out. Each form includes a description of its purpose. Read the instructions on the form carefully and submit it with the required documentation (when appropriate) to the Financial Aid and Scholarships Office.

All forms are in PDF format. You will need Adobe Acrobat to download and print the forms.

2012-13 Academic Year
Budget Appeal 2012-13	This form will be available in September 2012. This form is used to claim allowable expenses that you are incurring that are greater than the standard budget. Note: The processing time for budget appeals is 2 to 4 weeks.
Eligible Non-Citizen Statement 2012-13	Only use this form if you have been directed to do so by a UC Berkeley Financial Aid and Scholarships Office representative. If you are required to fill out this form you must bring it in person to our office.
U.S. Citizenship Confirmation 2012-13	This form is used when proof of U.S. citizenship is being required. Only use this form if you have been directed to do so by a UC Berkeley Financial Aid and Scholarships Office representative.
2011-12 Academic Year
Budget Appeal 2011-12 Deadline Has Passed	This form is used to claim allowable expenses that you are incurring that are greater than the standard budget. Update: This form is no longer available; the deadline to submit the form for spring semester 2012 was April 15, 2012.
Eligible Non-Citizen Statement 2011-12	Only use this form if you have been directed to do so by a UC Berkeley Financial Aid and Scholarships Office representative. If you are required to fill out this form you must bring it in person to our office.
