Undergraduate Application for Readmission Instructions

Begin by filling out these two forms (Readmission Application AND the Statement of Legal Residence):

Who must apply for readmission?
 

Undergraduate students who:

  1. Formally withdrew from the University
  2. Were absent for one or more semesters
  3. Are returning to UC Berkeley in the Limited status or Second Bachelors' programs

Application Submission Dates:

June 1
(Fall Semester) and  November 1 (Spring Semester)

Please check with your college for specific deadlines and related information. You should begin the process of readmission at least six weeks before the submission date. Check the Student Calendar  for registration information.

Students returning to the College of Letters and Science: Please click on this link  for College-specific instructions and checklist.

In order for the Office of Undergraduate Admissions to process your application you must:

  1. Clear all registration blocks.
  2. Complete the Application for Readmission. Incomplete applications will be returned, unprocessed.
  3. Attach a $60.00 non-refundable, non-transferable Application for Readmission fee. Make checks or money orders payable to UC Regents. PLEASE DO NOT SUBMIT CASH.
  4. Complete the Statement of Legal Residence (SLR). Click here for instructions. Print and attach the completed form, and attach a copy to your application.

College of Letters and Science students: To declare or change your major, click here

All other students: To change your College or major, you  must fill out a form from Registrar and petition to change. 

Attendance at other institutions:

If you have attended or are attending other educational institutions during your absence (including University Extension), you must:

  1. Send official transcripts from those institutions immediately to the address below. Your Application for Readmission may be delayed or denied if transcripts are not received.
  2. Notify the Office of Undergraduate Admissions if you enroll elsewhere subsequent to filing this application.

Official readmission after a Medical Withdrawal:

If the medical director of University Health Services signed your petition to withdraw from a term and UHS approval is required for readmission, contact the University Health Services to begin the process of readmission review. Allow at least one month for the review, which will be seeking evidence of your readiness to return to academic work.
